🧫 1. 貓 pemphigus foliaceus（B1658，⏱ 00:00–20:30，Abby → Chiara）
Abby 的描述（Chiara：描述得很好）： 貓（compound follicles、皮脂腺空泡均一）；大而半完整的 intracorneal 膿疱、跨多個毛囊，內含完整的嗜中性球＋單顆或成群的游離 acantholytic keratinocytes＋偶爾嗜伊紅球；局部變 subcorneal、侵入毛囊漏斗部；膿疱頂＝厚痂，變性嗜中性球與 acantholytic cells 夾在 orthokeratotic 角質層之間；活表皮：irregular hyperplasia、acanthosis、moderate spongiosis（基底／基底上）、局部嚴重嗜中性球 exocytosis。
Chiara 的補充：
- ⭐ Cling-ons：從膿疱頂「掛下來」的單顆或成群 acantholytic cells——subcorneal pustular dermatitis 的典型。
- ⭐ Multilayering：一波波的膿疱形成——最靠近表皮的是最新鮮的膿疱（嗜中性球最完整），上面一層角質、再一層舊膿疱（已成痂）、再角質……膿疱從 subcorneal 開始、往上移、再 re-cornify → 這種分層＋很寬的膿疱是 PF 的典型。
- 毛囊也有膿疱要提。細胞：嗜中性球＋嗜伊紅球（有些病例嗜伊紅球極多）；真皮混合（嗜中性球、肥大細胞為主）。
- Morph（Abby）：Severe, chronic（表皮增厚）, neutrophilic, intracorneal pustular dermatitis with acantholytic keratinocytes spanning multiple follicles → Chiara：病機上用 subcorneal（雖然此刻是 intracorneal）；multilayering 可留在描述；subcorneal、跨多毛囊、大量 acantholytic cells 三項一定要有——acantholytic cells 的數量、膿疱的形狀與大小，就是與其他 acantholytic subcorneal dermatoses 的區分點。
- 文獻：貓 PF 可伴血管變化／vasculitis——Carolina 找到 Bizikova／Falter 等 2019「Clinicopathological findings and clinical outcomes in cases of feline pemphigus foliaceus」（Chiara 確認就是這篇）。不是每例都有，但要知道。
🎯 教學重點（依逐字稿補）：
- Morph 照病機寫 subcorneal；subcorneal＋跨多毛囊＋acantholytic cells（“very many”） 是必備，multilayering 寫在描述即可。
- Acantholytic cells 的數量、膿疱的形狀與大小，是和其他 acantholytic subcorneal dermatoses 區分的依據。
- 毛囊內也有膿疱一定要提；膿疱裡可有嗜伊紅球，有些病例 “very eosinophil-rich”。
🧫 2. 豬 porcine juvenile pustular psoriasiform dermatitis（B65，⏱ 20:30–32:30）
- 物種推理（學員）：simple follicles、整片只有 3–4 個漏斗部、basket-weave → 不是馬牛（毛太少）→ 豬；但豬的皮脂腺空泡是不規則的（Chiara 也沒特別注意過——新學到）；皮下厚薄看部位。
- 表皮：大的 intracorneal 膿疱／痂（血、變性細胞、嗜伊紅球、嗜中性球），整片從表皮剝離（膿疱脆弱，可能正常）；diffuse moderate basket-weave orthokeratotic hyperkeratosis；moderate irregular hyperplasia；mild spongiosis。
- 真皮：嗜伊紅球為主（顆粒清楚）＋淋巴球；學員困惑的「一堆東西」＝被壓扁的紅血球在微血管裡（兩側內皮、中間血漿或擠扁的紅血球）。
- ⭐ Morph 的順序（Abramo 教授的原則，義大利名皮膚病理學家）：「pustular dermatitis 贏過 perivascular dermatitis」——有膿疱型態就把它放最前面：Severe, chronic, intracorneal pustular dermatitis with moderate interstitial to diffuse eosinophilic dermatitis。
- 疾病：豬會形成膿疱的病很少，名字裡就有 pustular → porcine juvenile pustular psoriasiform dermatitis：膿疱在表皮與毛囊；慢性化後出現 psoriasiform hyperplasia＝大小、深度相近、規則排列的 rete ridges（scalloped）——所以這裡的增生其實偏規則。病機不明、可能遺傳（juvenile）、通常數週自癒。
🎯 教學重點（依逐字稿補）：
- 物種判讀：simple follicles＋毛囊極稀少＋basket-weave → 豬；豬的皮脂腺空泡不規則（Chiara 也說是 “a new thing”）。
- 學員原本把 eosinophilic dermatitis 放前面、膿疱放後面（“with … intracorneal pustules”）→ 依 Abramo 原則改成 pustular dermatitis 開頭，真皮變化放後面。
- 微血管內被擠扁、拉長的紅血球不要誤判成別的東西（兩側是內皮細胞、中間是血漿或紅血球）。
- 慢性期 rete ridges 大小、深度相近、呈 scalloped → 增生其實是「規則的」psoriasiform hyperplasia。
🧫 3. 貓 pustular dermatophytosis（B819，⏱ 32:30–40:00，Ulli）
- 貓（compound follicles＋皮脂腺）；嚴重廣泛 intracorneal 膿疱＋serocellular crust、多發潰瘍；毛囊：luminal folliculitis（嗜中性球）＋有隔的絲狀構造＝菌絲、毛囊上皮 spongiosis；痂與膿疱裡也有長的有隔菌絲、分層的痂含 acantholytic cells 與嗜中性球；表皮基底／棘層嚴重 spongiosis；真皮「很無聊」。
- Morph（Ulli）：Severe intracorneal pustule formation with serocellular crusting containing acantholytic keratinocytes and septate non-pigmented fungal hyphae and spores extending into the follicular lumen → pustular dermatophytosis ✅。
- Chiara 修正：寫 intracorneal pustular dermatitis（要有 pattern 名）；serocellular crust 是次發、放後面；hyphae 一定要進 morph——⭐ morph 的哲學：pattern ＋「讓你在同 pattern 裡分出是哪個病的元素」（膿疱位置＋菌絲＝pustular dermatophytosis；拿掉菌絲就跟 PF 分不開）。
- 學員：「這樣寫 morph 讓我對考試很不安」→ Chiara：「morph 沒有唯一寫法，連考官給分時都會無止境地爭論——這不是你的問題，是我們共同的問題。」
- Chiara 修正後的 morph（依逐字稿補）：Severe intracorneal pustular dermatitis with acantholytic keratinocytes and (intralesional) septate fungal hyphae；serocellular crusting 拿掉或放到最後。
🎯 教學重點（依逐字稿補）：
- Morph 裡要「命名 pattern」：寫 pustular dermatitis，不要只寫 pustule formation。
- 次發變化（serocellular crust）不放在 morph 前段。
- 菌絲是讓 pustular dermatophytosis 與 pemphigus foliaceus、pemphigus vulgaris 等同 pattern 疾病分開的關鍵元素，一定要進 morph。
🧫 4. 犬 Leishmania 相關 pustular dermatitis（case 9，⏱ 40:00–52:30，Abi Kerr）
- 犬有毛皮膚；pattern：pustular/vesicular。角質：basket-weave orthokeratotic＋膿疱處局部 parakeratosis；表面與膿疱內有游離毛幹；intra／subcorneal 膿疱含嗜中性球、角質碎片、毛幹、少數 acantholytic cells；moderate irregular hyperplasia、spongiosis、局部細胞內水腫、局部嗜中性球 exocytosis。
- 真皮：superficial perivascular→interstitial、混合（肥大細胞、嗜中性球、淋巴球）、局部至瀰漫的表淺出血、輕度水腫；毛囊附近的嗜中性球＝中真皮神經叢的發炎，不必另寫 periadnexal；深層「擴張」的血管其實只是大血管；汗腺輕度擴張。
- Morph（Abi）：Severe, acute, diffuse, subcorneal pustular dermatitis with neutrophils, few acantholytic cells and free hair shafts ✅（neutrophilic 或 containing neutrophils 擇一；exocytosis 不進 morph）。
- ⭐ 要寫 acantholytic cells 的數量：免疫媒介的 pustular dermatosis 預期 acantholytic cells 較多（numerous），細菌／黴菌性較少（few）——文獻有描述，不是 pathognomonic 但有幫助。
- 學員問毛囊是否萎縮：切片定向不佳、看不到下段；有些像萎縮的細索，但沒有臨床脫毛資訊時 Chiara 不會評論。
- ⭐ 答案：Leishmania 相關的 pustular dermatitis——Chiara 找了很久沒看到含 amastigotes 的巨噬細胞，IHC 陰性、石蠟塊 PCR 也可陰性，這在 Leishmania pustular dermatitis 並不意外；這些狗通常血清高效價、全身有病。在義大利，看到 pustular dermatitis 一律把 Leishmania 放進鑑別。
